In order to complete your Annual Report, you need to log in to the portal first. Directions on how to submit an Annual Report can be found in the righthand column of this page. Directions will also be included in the email you receive from us when your report is due.
Licensed organizations will receive an email from SMRC when their annual reports are due. The email will have their License #, which is necessary to complete the online form.
Master Trainers and T-Trainers are required to submit a certification form and annual reports online.
- Trainer Certification Form
- New Master Trainers and T-Trainers, as well as those adding an additional program, are required to submit their Trainer Certification Form before facilitating a Leader training.
- Annual Reports
- Master Trainers and T-Trainers are required to submit annual reports of their training activity for the previous calendar year in January of the following year. You will receive an email in January with instructions. Do not attempt to submit your 2017 Annual Report until your receive an email from us with instructions.
