Chronic Disease Self-Management Master Training

The CDSMP Master Trainer training will be offered at locations in Palo Alto, CA. Please see details about event location when you sign up for a specific training session. Additional details will be provided when the class is confirmed.
$1,800 FULL training fee per professional (examples include employee with a role of program coordinator, health educator, social worker, researcher, diabetes educator, nurse, doctor).
$1,000 FULL training fee for a lay person with chronic disease (examples include a volunteer for an organization who is donating time to deliver the self-management programs).
How Our Programs Work
All workshops and trainings are designed to be facilitated by 2 people. It is strongly suggested that health professionals bring a lay person with a chronic disease with them. Each trainee receives a detailed CDSMP Leader's Manual, and a copy of the workshop's textbook and audio CDs (if applicable). Those being trained as Master Trainers (trainers of leaders) will also receive a trainer's manual. In order to become a certified Master Trainer you will need to attend training, and facilitate 2 participant workshops (each workshop has 6 sessions) within 12 months of being trained. After you have facilitated 2 workshops and submitted the required paperwork for certification as a Master Trainer, you are authorized to train new leaders.
In order to train in or deliver any of our self-management programs, your organization will need a license for each program you plan to deliver. Before you register for training, find out if your organization has a license. If your organization has a license, you will need to provide your license information. If your organization does not have a license, you can purchase a license during the registration process. However, you will need to know which type of license your organization needs. When you register for training, you can purchase a single or multi program license. The term of a license is 3 years. If you plan to offer more than 110 CDSMP workshops over the term of the license, or are in need of a multi site license, you will need to contact us for more details at email@example.com. How to Pay for Training
We will take credit card, check, purchase order, or wire transfer. However, your spot will not be guaranteed until we reach minimum enrollment of sixteen and your payment is received. Credit card payments will be processed as soon as we reach minimum enrollment. If you are using any other form of payment, your spot will be guaranteed by email when your payment is received. If the training is full when your payment is received, you will be placed on the waiting list.
What's Our Cancellation Policy?
CANCELLATION POLICY / REFUND DEADLINE (with FULL tuition): 3 weeks prior to training date! Training slots are limited; first come, first served when full payment is received. No refunds after the deadline. (Refunds available only if class is canceled by Stanford.) We will not be able to confirm the training date until we have 16 registered. * Do NOT make travel arrangements until you receive a class and enrollment confirmation and training session details letter. *
How to Register For Training
If you know which training you want to register for AND understand licensing, then you are ready to register. You can click on this link to proceed to registration pages, Register Now Link. This link will take you to the registration form and payment screens. Once you register, we will send a Registration Confirmation email. However, DON’T Confirm TRAVEL ARRANGEMENTS UNTIL YOU GET AN EMAIL FROM US INSTRUCTING YOU TO DO SO. A separate email will be sent once we confirm the training session and your spot in that class.
